Immunization Requirements
An immunization record, certified by a physician or other public health personnel is required. Student’s up-to-date immunizations should include the following: |
Diphtheria, Tetanus, Pertussin (DTP, DtaP, Td) |
| Less than age 7 |
4 doses including one received on or after 4th birthday |
| Ages 7+ |
3 doses DTP/DtaP/Td vaccine including one received on or after 4th birthday and 1 dose of Td within the last 10 years.

| Polio (OPV, IPV) |
3 doses polio vaccine including one received on or after 4th birthday (not required for student 18 years of age or older)

| Mumps, Measles,Rubella (MMR) |
1 dose of MMR vaccine received on or after 1st birthday and 1 additional dose of measles vaccine (this may be received as part of a second MMR)
|
| Haemophilus Influenza (HibCV) |
Series completed + booster (ages 5+, HibCV not required), or 1 dose after 15 months
|
| Hepatitis B (HepB) |
3 doses by age 5 if born on or after 09-02-92; or at age 12 if born between 9-02-88 and 09-01-94(inclusive).
|
| Varicella (Chicken Pox) |
1 dose of varicella vaccine if born on or after 09-01-94; or age 12 if born between 09-02-88 and 09-01-94(inclusive).

| TB Test/Results |
The District shall comply with screening recommendations made by local health authorities.
